One copy of this application form (typed) must be received by the Development Foundation Office by March 15, 2013. Copies of completed form may be sent via e-mail to julieb@ndsualumni.com. Applicants will be notified of funding decisions before April 30, 2013.
Grants provided to successful applicants must be expended within 18 months of award notification.
