One copy of this application form (typed) must be received by the Development Foundation Office by February 29, 2012. Copies of completed form may be sent via e-mail to joann@ndsualumni.com. Applicants will be notified of funding decisions before March 30, 2012.
Grants provided to successful applicants must be expended within 18 months of award notification.
